Sampler of Agency Self-Assessment Activities Related to Person-Centered Planning

The Bureau of Planning and Service Design of the New York State Office for people with Developmental Disabilities (formerly the New York State Office of Mental Retardation and Developmental Disabilities) developed a reference document called the "Sampler of Agency Self-Assessment Activities Related to Person Centered Planning." This sampler was developed in response to inquiries received from agencies statewide regarding methods for self-monitoring organizational activities and outcomes related to person centered approaches. Most providers of developmental services and supports have been implementing person centered approaches for a number of years, and many of these have indicated to us that they are interested in monitoring the results they are getting, that is, how they are doing at being "person centered." Often they wish to establish benchmarks to which they can compare future progress.
This sampler is a resource describing a variety of approaches and materials that can be used by agencies in order to gather data regarding progress in organizational change and improvement in point-of service performance that they have achieved in terms of the delivery of individualized supports and services.
The approaches presented in the sampler are not exhaustive. Rather, they reflect managerial practices intended to gather information about program and service processes, whether individualized services and supports are being developed and delivered, and the outcomes that have resulted. The intent of these approaches is to provide managers and other key decision-makers with the useful information upon which continuous improvement efforts may be based. Although particular documents, such as resource materials or published organizational standards are identified in the sampler, their use is illustrative, and in many instances other resources or standards could be employed in combination with the several methods described.
