This guidebook is for family caregivers of a person with a developmental disability. It is designed as a workbook to help guide family caregivers' thoughts and discussions about the future. It also serves as a resource for other family members and service providers to help support the family caregiver as they go through the planning process.

Each section has introductory pages, but most of the guide book is made up of worksheets and questionnaires that walk people through the planning process. This workbook follows the tenets of most models of Person Centered Planning.
